Qualifications

• MSW degree required.
• Licensed clinical social worker (LCSW) in good standing.
• Licensed social worker (LSW) in good standing.
• Driver's license required.
• Proof of auto insurance required.
• Hospice experience preferred.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plan of care addressing family/caregiver needs.
• Interpret and communicate pt/family faith and cultural traditions.
• Educate patient and family to overcome barriers and meet fundamental needs.
• Document psychosocial assessments, financial assessments, and MSW interventions in EMR.
• Collaborate with IDT and ensure care plans reflect palliative philosophy.
