Qualifications

• Master’s degree in Social Work required.
• LCSW preferred.
• If practicing in SC, must meet state licensure.
• At least 1 year hospital/health setting experience preferred.
• Expertise with Data Management Tools.
• Adherence to NASW Code of Ethics.

Responsibilities

• Interviews patient and support system to complete psychosocial evaluation.
• Provides counseling and education to patient and support system.
• Assists patients and families in coping with hospitalization, disability, or terminal illness.
• Collaborates with multidisciplinary team to plan care addressing psychosocial needs.
• Serves as a resource to staff regarding emotional and psychosocial aspects of illness.
• Aids and facilitates team in integrating psychosocial considerations into care plans.
• Documents in medical records per departmental standards.
• Develops and maintains accurate case records for referred patients.
