Job summary

Advocate Health, based in Monroe, NC, seeks a Master’s level Social Worker for hospice bereavement services. This full-time role involves psychosocial evaluation, counseling, and coordination with a multidisciplinary team to support patients and families.

You will assess needs, connect with community resources, and contribute to care planning while adhering to NASW ethics. M-F, 40 hours weekly with comprehensive benefits.

Qualifications

• Master's degree in Social Work from a program accredited by the council in Social Work Education required.
• LCSW preferred.
• If working in the state of South Carolina must meet and maintain license requirements as outlined by the state.
• At least 1-year professional hospital/health setting experience preferred.
• Expertise with Data Management Tools.
• Adherence to National Association of Social Workers Code of Ethics.

Responsibilities

• Interviews patient and social support to complete psychosocial evaluation.
• Provides counseling and education to patient and social support systems.
• Collaborates with multidisciplinary team to plan care addressing psychosocial needs.
• Serves as a resource to hospital staff regarding emotional and social aspects of illness.
• Intervenes in crisis situations using crisis intervention theory and skills.
• Completes psychosocial assessments for post-hospital care as needed.
• Assesses appropriate level of care with Clinical Care Management team.
• Participates in multidisciplinary care planning meetings.
• Identifies community resources and referrals.
• Reports suspected abuse/neglect/exploitation and coordinates with Department of Social Services.
