Job summary

Advocate Health in Monroe, NC seeks a Social Worker to conduct psychosocial evaluations, provide counseling, and help plan care with a multidisciplinary team. The role emphasizes crisis intervention, community resource referrals, and coordination across the care continuum.

A Master’s in Social Work is required, with LCSW preferred, and hospital experience is valued. The position is full-time with standard M-F hours and a regular schedule.

Qualifications

• Master's degree in Social Work from a program accredited by the Council on Social Work Education.
• Licensure as LCSW preferred; SC license if working there.
• At least 1 year of professional hospital or health setting experience preferred.
• Experience with Data Management Tools and ethics per NASW code.
• Commitment to NASW Code of Ethics and professional standards.

Responsibilities

• Interviews patients and support systems to complete psychosocial evaluations.
• Provides counseling and education to patients and families.
• Develops care plans with multidisciplinary teams to meet psychosocial needs.
• Serves as a hospital resource on emotional, social, and psychosocial aspects.
• Assists and facilitates crisis intervention as needed.
• Completes psychosocial assessments for post-hospital care.
• Collaborates with Clinical Care Management to determine level of care.
• Maintains accurate case records and ensures proper documentation.
