Job summary

Advocate Health is seeking a Part-time Medical Social Worker in Mecklenburg County to assess psychosocial needs, provide counseling, and coordinate with a multidisciplinary team for post-hospital planning.

Master's degree in Social Work is required with LCSW preferred; 1+ year hospital experience is desirable. Opportunities include involvement with clinical care management and community resource referrals.

Qualifications

• Master's degree in Social Work (MSW) required.
• LCSW preferred for licensure.
• At least 1 year of professional hospital/health setting experience preferred.

Responsibilities

• Interviews patient and social support to complete psychosocial evaluation.
• Provides counseling and education to patient and social support; assists coping with hospitalization, disability, illness.
• Collaborates with multidisciplinary team to implement care plans addressing psychosocial needs.
• Serves as a resource to staff on social, emotional and psychosocial aspects of illness.
• Intervenes in crisis situations using crisis intervention skills.
• Documents psychosocial assessments and plans in the medical record.
