Job summary

Affinity Health Management is seeking a Medical Social Worker (MSW) to implement standards of care for hospice patients and their families. The MSW collaborates with the Interdisciplinary Group to assess needs and provide psychosocial support aligned with the plan of care and professional judgment.

The role involves psychosocial assessments, care planning, education on directives, crisis intervention, and coordination with care teams to ensure compassionate, comprehensive end-of-life support.

Qualifications

• MSW degree from CSWE-accredited school.
• Minimum of 1 year experience in health care; hospice experience preferred.
• Hospice experience preferred.
• Demonstrates good verbal and written communication and organizational skills.
• Background checks and references will be requested and reviewed.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers and report findings to the care team.
• Cary out social evaluations and plan interventions based on evaluations.
• Educate on advanced directives and cultural considerations.
• Develop and participate in the individualized plan of care with patient/family.
• Provide crisis intervention and support through the interdisciplinary group.
• Maintain accurate clinical notes and coordinate care with physicians.
