Qualifikationen

• MSW from a CSWE-accredited program.
• Minimum 1 year experience in health care, hospice experience preferred.
• Must hold an active MSW license (NJ).

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Provide crisis intervention and counseling as needed.
• Educate patient/family on cultural considerations and advance directives.
• Participate in development of the individualized plan of care with patient and family.
• Assist the team in understanding social and emotional factors related to health problems and end-of-life issues.
• Plan with family for funeral, financial, legal, and health care decision responsibilities.
• Attend interdisciplinary group meetings and coordinate with community agencies to support care.
