Job summary

Ascension At Home, in partnership with Compassus, is seeking a Hospice Social Worker to address the psychosocial needs of patients and families. You will work with the IDT to develop care plans, provide counseling, and facilitate advance planning and community resource coordination in Indiana.

Ideal candidates hold an MSW (CSWE-accredited) with healthcare experience and state licensure; bachelor’s degree in social work with supervision options may be considered in certain locations.

Qualifications

• MSW from a CSWE accredited program with at least 1 year healthcare experience.
• Bachelor’s degree in social work or related field with 1 year healthcare experience; MSW available for supervision.
• State licensure as required and applicable for the state in which you practice.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent.
• Submit documentation to facilities after patient visits; ensure timely submission.
• Participate as a member of the Interdisciplinary Team (IDT) for care planning.
• Provide psychosocial support and counseling; coordinate with external resources as needed.
• Identify intervention needs for other IDT members; communicate patient/family needs clearly.
• Maintain comprehensive care plans reflecting problems, goals and interventions.
• Complete initial and ongoing psychosocial assessments; address financial, living arrangements, caregiver stressors, and bereavement needs.
