Job summary

Hospice Care Center in Chattanooga seeks a dedicated Licensed Medical Social Worker (MSW) to provide compassionate social work services to hospice patients and families. You will assess psychosocial needs, offer counseling, and collaborate with the care team to support emotional well‑being during end‑of‑life care.

Schedule: Monday through Friday, 8am–4:30pm. Candidates must hold an MSW (or BSW with MSW supervision), be licensed, and have at least 1 year in healthcare or hospice settings.

Qualifications

• Master of Social Work from an accredited school.
• Bachelor of Social Work (must be supervised by MSW).
• Licensed Social Worker by the State Board of Social Work.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle.
• Minimum one year experience as a social worker in healthcare; hospice preferred.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement individualized social work plans and collaborate with the Bereavement Coordinator.
• Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide empathetic counseling to patients and families.
