Gentiva Hospice is seeking a compassionate Hospice Social Worker (MSW) to join our interdisciplinary team in Tennessee. You will perform psychosocial assessments, care planning, counseling, and connect patients and families with resources for end-of-life care.

The role relies on empathy, cultural sensitivity, and ethical judgment, collaborating with physicians, nurses, chaplains, and other team members to support the patient-family unit with dignity and peace of mind.

Formación

• MSW from a CSWE-accredited program.
• Current licensure as required by the state of employment.
• Minimum 1 year of social work experience in healthcare or hospice.
• Knowledge of psychosocial dynamics of illness, grief, loss, and end-of-life care.
• Ability to provide empathetic support and maintain boundaries.
• Strong communication skills and cultural sensitivity.

Responsabilidades

• Serve as psychosocial support expert on the interdisciplinary team.
• Perform assessments and care planning based on patients' and families' needs.
• Deliver counseling, crisis intervention, and resource support.
• Collaborate with care team to support patient-family well-being.
• Educate families on hospice services and advance care planning.
• Assist with referrals and navigation of community resources.
• Document psychosocial changes and update care plans as needed.
• Uphold ethical standards and professional judgment in end-of-life care.
