Job summary

Hospice of Chattanooga, Inc. seeks a Licensed Medical Social Worker (LMSW/MSW) to provide counseling and psychosocial support to hospice patients and families. This patient-centered role focuses on emotional well-being and care planning, with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m.

Qualifications

• MSW degree from an accredited school.
• Licensed Social Worker (State Board) required.
• At least 1 year in healthcare social work; hospice experience preferred.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager.
• Evaluate medical needs, home situations, resources and community supports.
• Develop personalized social work plans in collaboration with the Bereavement Coordinator.
• Join interdisciplinary meetings to shape care plans and address psychosocial stress.
• Provide empathetic counseling to patients and families.
