Job summary

SGMC Health is looking for a dedicated Hospice Social Worker to join our Interdisciplinary Team. You will provide essential services to support patients and their families during terminal illness, ensuring emotional and economic needs are met.

This full-time position requires a Master of Social Work, LMSW licensure, and teamwork. Ideal candidates have experience in hospice care and a passion for compassionate service.

Qualifications

• Master of Social Work degree from an accredited school required.
• LMSW licensure by the State of Georgia; LCSW preferred.
• Minimum of two years varied Social Work experience preferred.
• Understanding of hospice philosophy and interdisciplinary teamwork.
• Strong skills in psychosocial assessment and patient care.

Responsibilities

• Provide supportive services to meet patient/family needs.
• Assist with bereavement support as needed.
• Participate in grief counseling services for hospice staff.
