Job summary

Heart’n Soul Hospice in Memphis, TN seeks a licensed social worker to join our compassionate care team. LMSW is required; LCSW preferred. This full-time role focuses on psychosocial assessment, counseling, advance directives, and connecting families with community resources.

Responsibilities include on-call duties, documentation, and coordination with care teams to ensure patient-centered support while meeting regulatory requirements.

Qualifications

• Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and ongoing auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.
• LMSW required; LCSW preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs.
• Provide individual patient/family counseling, crisis intervention, and information for advance directives and related issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments consistent with education and competence.
• Perform on-call responsibilities and provide on-call service to patients/families.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws and Medicare regulations.
