Job summary

Compassus in Wisconsin seeks a Hospice Social Worker to deliver psycho-social care and counseling, integrated with the interdisciplinary team. You will assess needs, coordinate with care teams, and help patients and families navigate benefits, planning, and placements while honoring the Compassus values of compassion and integrity.

Qualifications

• MSW CSWE-accredited degree with at least 1 year healthcare experience.
• Bachelor's in social work considered with available MSW supervision.
• State licensure as required for practice in the state.

Responsibilities

• Explain hospice services and Medicare benefits and obtain informed consent.
• Provide psycho-social assessments and counseling to patients and families.
• Collaborate with the IDT to coordinate care and plan of care.
• Maintain comprehensive care plans reflecting problems, goals, and interventions.
• Support advance planning and end-of-life decisions; comply with policy.
