Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families, collaborating with interdisciplinary teams. Hospice experience required and a sign-on bonus is available to eligible applicants.

Role focuses on resource support, short-term counseling, crisis intervention, and care coordination. Licensure in Illinois and a Master’s degree in Social Work are required; driver’s license and auto insurance may be needed for home visits.

Qualifications

• Master's Degree in Social Work required.
• Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW) licensure required (IDFPR).
• Hospice experience of 1+ years is required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation.
• Develops a bereavement plan of care to address family member/caregiver needs.
• Interprets and communicates pt/family faith and culture traditions; documents psychosocial assessments and interventions in EMR.
• Implements social work plan to enhance family systems, maximize benefits, and support patient/family coping; evaluates effectiveness.
