Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families and to coordinate care with an interdisciplinary team. The role emphasizes hospice-focused interventions, crisis support, and resource linking to maximize patient and family well-being.

Applicants must have a Master’s in Social Work, hold LSW/LCSW licenses in Illinois, a valid Driver’s License, and 1+ year of hospice experience. A sign-on bonus is offered.

Qualifications

• Hospice 1+ years experience required.
• Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW) licensure per IDFPR (IL)
• Driver's License - SOS
• Proof of Auto Insurance - varies

Responsibilities

• Perform psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plan of care addressing family caregiver needs.
• Educate patients and families, addressing barriers and learning needs.
• Document assessments and interventions in the EMR in a timely manner.
• Collaborate with IDT to implement and adjust social work plans of care.
