Qualifications

• Master's degree in Social Work is required.
• Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW) licensure in Illinois.
• Hospice experience of 1+ year is required.
• Ability to work as part of a team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs related to diagnosis and care.
• Develop a bereavement plan of care addressing family needs.
• Educate patients and families in culturally sensitive ways and overcome barriers to learning.
• Document assessments and interventions in the electronic medical record.
• Collaborate with IDT to develop and implement social work plans of care.
