Job summary

Qualifications

• MSW required; hospice experience preferred
• LSW or LCSW licensure in appropriate jurisdiction required
• Valid driver’s license and auto insurance as applicable

Responsibilities

• Provide psychosocial assessments of patients and families to identify needs and strengths.
• Develop bereavement plans addressing family/caregiver needs.
• Educate patients and families about learning needs and cultural considerations.
• Document assessments and social work interventions in the electronic medical record.
