Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families and coordinate care with interdisciplinary teams.

The role requires Hospice experience of 1+ year, a Master's Degree in Social Work, and licensure (LSW/LCSW) where applicable. A sign-on bonus is offered, and the position supports compassionate, patient-centered care across our Illinois service areas.

Qualifications

• Master's degree in Social Work required.
• LSW/LCSW licensure as applicable (IL or state where practicing).
• 1+ year Hospice experience required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care for families.
• Educate patients and families respecting beliefs and culture.
• Document assessments and interventions in EMR.
• Collaborate with interdisciplinary team to implement care plans.
