• Licensure as LSW or LCSW preferred.
• Hospice experience of 1+ year required.

Responsibilities

• Perform psychosocial assessments of patients and families.
• Develop bereavement plan of care.
• Communicate patient/family beliefs and culture to the team.
• Educate patients and families to overcome barriers and meet needs.
• Document assessments and interventions in EMR.
• Collaborate with IDT to develop and adjust care plans.
• Support patient decisions and advance care planning.
• Address late-life care and hospice benefits and eligibility.
