Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs in patients and families through assessment and to provide interventions ranging from resource support to short-term counseling. The role collaborates with interdisciplinary teams and may require hours to vary with census and program needs.

Qualified applicants will hold a Master’s in Social Work and licensure as an LCSW or LSW, with valid Illinois credentials, and will have hospice experience.

Qualifications

• Master’s Degree in Social Work is required.
• Licensure as LCSW or LSW is required per IDFPR.
• Driver’s License required by SOS as applicable in IL.
• Proof of Auto Insurance required by issuer.
• Hospice experience is preferred/required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care for family needs.
• Communicate patient/family beliefs and culture to the team.
• Educate patients and families to overcome barriers and meet needs.
• Document assessments and interventions in EMR.
• Collaborate with IDT to develop social work plan of care.
• Support decisions and communications of life care preferences.
• Identify and respond to indicators of imminent death and care needs.
