Job summary

Mercy Health Home Care and Hospice by Compassus in Kentucky seeks a compassionate Hospice Social Worker to support patients and families, coordinating psychosocial care with the interdisciplinary team to meet goals of comfort and quality of life.

The role emphasizes adherence to ethical conduct, assessment, care planning, and facilitating advanced directives, placements, and community resources, while upholding Compassus values.

Qualifications

• MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
• Bachelor's degree in social work with 1 year healthcare experience may be considered for locations with MSW supervision.
• State licensure as required for the state of practice.

Responsibilities

• Explain hospice services and Medicare benefits; obtain informed consent and election documents.
• Submit documentation to facilities after visits; ensure timeliness per policy.
• Participate as a member of the IDT and contribute to the plan of care.
• Provide psycho-social support and counseling to patient and family; coordinate with external resources.
• Identify need for intervention by other IDT members.
• Communicate patient/family needs to IDT effectively.
• Coordinate psycho-social care with IDT for continuity and quality.
• Maintain comprehensive, current care plans reflecting problems, goals, interventions.
• Respect family environment and nondiscriminatory practice across age, race, religion, and disability.
• Conduct initial and ongoing psycho-social assessments; address financial, placement, caregiver stressors, bereavement needs.
• Assist in placement in long-term care and monitor adjustment.
• Participate in long-term care care planning meetings.
• Support advance planning including living wills/POA, funeral arrangements, and memorial services.
• Perform other duties as assigned.
