Job summary

Caretenders Hospice, part of the Optum family, seeks a Social Worker to provide medical social services under physician direction and an interdisciplinary team. The role focuses on understanding social factors influencing health and helping patients cope.

The position supports creation of an integrated Plan of Care and uses community resources to assist patients and families as needed. Training and development opportunities are available within a supportive environment.

Qualifications

• Master’s Degree from a school of Social Work accredited by the CSWE.
• Current Driver’s License, vehicle insurance, and access to dependable transportation.

Responsibilities

• Assist the patient, family, physician and health care team to understand social factors affecting health.
• Contribute to the daily development of a comprehensive Plan of Care.
• Instruct team members on community resources available to assist patients.
