Job summary

Compassus is seeking a Hospice Social Worker to provide psychosocial assessments, counseling, and support to patients and families. You will collaborate with the inter-disciplinary team to coordinate care, facilitate advance planning, and ensure documentation aligns with policy.

The role requires an MSW with healthcare experience or a BA in social work with supervision availability, and state licensure as required. You will operate within Compassus values of compassion and integrity.

Qualifications

• MSW from a CSWE-accredited program with at least 1 year healthcare experience.
• Bachelor's degree in social work with 1 year healthcare experience will be considered for locations with MSW supervision.
• State licensure as required for the state you practice.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent and election documents.
• Submit documentation to facilities promptly after patient visits per policy.
• Participate as a member of the IDT; develop and implement the plan of care.
• Provide psycho-social support and counseling to patients and families; coordinate with external counseling resources when needed.
