Job summary

UnitedHealth Group is recruiting for a Social Worker within the Mederi Caretender's Hospice program. The role provides medical social services under physician direction to help patients and families understand social and emotional factors related to health, and to assist in coping strategies.

You will participate in an interdisciplinary team to develop an integrated Plan of Care and connect patients with community resources as needed. This position offers a competitive salary and benefits.

Qualifications

• Master's Degree from a school of Social Work approved by the CSWE.
• Current CPR certification or ability to obtain within 90 days.

Responsibilities

• Assists patients and families to understand social and emotional factors affecting health status.
• Contributes to development of a comprehensive Plan of Care with the health care team.
• Instructs team members on available community resources for patient support.
