Job summary

Caretenders Hospice, part of UnitedHealth Group, is hiring a Social Worker to provide medical social services under physician direction, helping patients and families understand social and emotional factors affecting health. Primary responsibilities include supporting care planning with the interdisciplinary team and connecting patients with community resources.

Requirements include MSW from CSWE-accredited program and a valid driver's license; preferred 1+ year health care social work, CPR

Qualifications

• Master’s Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation

Responsibilities

• Assist the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instructs health care team members on community resources available to assist patients on a as needed basis
