Job summary

UnitedHealth Group is seeking a Social Worker to provide medical social services under the direction of a physician and the Interdisciplinary Group, helping patients and families understand social and emotional factors related to health status and coping strategies.

You will help develop a comprehensive Plan of Care with the health care team, educate staff about community resources, and support patients on their daily journey toward better health within a supportive, inclusive environment.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Current Driver's License and access to reliable transportation.
• Preferred: 1+ year of health-care social work; CPR certification; licensed Social Worker in state of residence.

Responsibilities

• Assists patients, families, physician, and care team to understand social and emotional factors affecting health.
• Contributes to daily development of a comprehensive Plan of Care with the care team.
• Instructs staff on available community resources to assist patients.
