Job summary

UnitedHealth Group’s Assured Hospice, part of LHC Group, seeks a Social Worker to provide medical social services under physician and Interdisciplinary Group guidance, addressing social and emotional factors impacting patient health.

Responsibilities include assisting patients and families to understand factors shaping health, contributing to an integrated Plan of Care, and educating the care team about community resources.

Qualifications

• Master’s Degree in Social Work required.
• Valid driver’s license and access to a dependable vehicle or transportation.
• WA state credential required if not licensed as a social worker.

Responsibilities

• Assists patients and families in understanding personal, emotional and social factors affecting health.
• Contributes to development of a comprehensive Plan of Care with the interdiscplinary team.
• Instructs care team on community resources available for patients.
