Job summary

Compassus is seeking a Hospice Social Worker in New Hampshire to provide psychosocial support to patients and families as part of the interdisciplinary team. The role emphasizes compassionate care, ethical practice, and collaboration with facilities, while maintaining comprehensive care plans and coordinating community resources.

MSW from an accredited program with healthcare experience is required, with state licensure as applicable.

Qualifications

MSW from CSWE-accredited program with at least 1 year healthcare experience.
Bachelor's in social work and 1 year healthcare experience considered where MSW is available to supervise.
State licensure as required in the state where you practice.

Responsibilities

Explain hospice services and Medicare benefits to patients and families; obtain Informed Consent and Election of Benefits documents.
Submit documentation to facilities after patient visits; ensure timely completion per policy.
Participate as a member of the IDT and develop/implement the plan of care.
Provide psychosocial support and counseling to patients and families; coordinate external counseling resources as needed.
Maintain comprehensive care plans reflecting current problems, goals, and interventions.
Assist in placement in long-term care and monitor patient adjustment.
Participate in care planning meetings at long-term care facilities.
Support advance planning including living will/POA documents and funeral arrangements.
