Job summary

Compassus seeks a Hospice Social Worker to provide psychosocial assessments, counseling, and care coordination for patients and their families within our hospice program. You will work with the Interdisciplinary Team to plan and execute individualized care, ensuring compassionate, values-driven service.

The role emphasizes counseling, coordination with community resources, documentation, and adherence to ethical standards as part of a compassionate, inclusive care team.

Qualifications

• MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
• State licensure as required for practice.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent and election documents.
• Submit documentation to facilities per policy and on time.
• Participate as a member of the IDT and develop and implement the care plan.
• Provide psycho-social support and counseling to patients and families; coordinate with external counseling resources as needed.
• Identify need for intervention of other IDT members and communicate needs to the team.
• Coordinate psycho-social care to ensure continuity and quality of care.
• Maintain updated care plans reflecting problems, goals, and interventions.
• Support advance planning including living wills/POA documents and end-of-life arrangements.
