Compassus is hiring a Hospice Social Worker in Arizona. You will provide psychosocial support, counseling, and coordination with the interdisciplinary team to improve coping for patients and families.

You’ll manage care plans, assist with advance directives, and connect families with resources while upholding Compassus values. Qualifications include an MSW from a CSWE-accredited program (or BSW with supervision eligibility) and state licensure as required, with patient-centered care and

Qualifications

• MSW from CSWE-accredited program with at least 1 year of healthcare experience.
• Bachelor’s in social work with 1 year healthcare experience may qualify where MSW supervision is available.
• State licensure as required for the practicing state.

Responsabilités

• Explain hospice services and Medicare benefits to patients and families.
• Submit documentation to facilities after patient visits per policy.
• Participate as a member of the IDT and support care plan development.
• Provide psycho-social support and counseling to patients and families.
• Collaborate with IDT to coordinate psycho-social care.
• Maintain comprehensive care plans reflecting current problems and goals.
• Facilitate advance planning including living will/POA and funeral arrangements.
