Job summary

St-Croix Hospice- seeks a Licensed Social Worker to implement standards of care in medical social work services. You will assess psychosocial status of patients and families, provide crisis intervention, educate on advance directives, and coordinate with the care team to support meaningful end-of-life planning.

You will maintain records, participate in interdisciplinary meetings, and assist with bereavement programs and community referrals, while ensuring HIPAA compliance and professional growth.

Qualifications

• Bachelor's degree in social work from CSWE-accredited program.
• Master's degree in social work preferred.
• Licensed to practice social work in the state served.
• One year of health-care social work experience; hospice preferred.
• CPR certification current.
• Ability to pass DHS background check.
• Valid state driver’s license and reliable vehicle.

Responsibilities

• Assesses psychosocial status of patients and families facing terminal illness.
• Provides crisis intervention and coping support.
• Educates on advance directives and coordinates with care team.
• Maintains confidential clinical records and HIPAA compliance.
• Participates in interdisciplinary team meetings and care planning.
• Assists Bereavement program and planning for family support.
