Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop a bereavement plan of care for family members/caregivers.
• Educate patients and families about learning and cultural beliefs affecting care.
• Document assessments and interventions in the EMR.
• Develop and implement social work plans of care with the IDT.
