Job summary

Heart to Heart Hospice in Texas is seeking a Community Education Representative to drive hospice referrals by engaging physicians, hospitals, and senior-care facilities. This role focuses on building awareness of Heart to Heart Hospice House and community resources.

You will develop territory plans, manage relationships, and coordinate with the Hospice House team to support patient admissions while meeting growth goals.

Qualifications

• 1–3 years of progressive healthcare sales experience preferred.
• Knowledge of Medicare Hospice Benefit and business management.
• Excellent time management and ability to work autonomously.
• Valid driver's license and reliable transportation.

Responsibilities

• Call on physicians, area hospitals, skilled nursing facilities, assisted living facilities or other senior living settings to promote hospice referrals and Heart to Heart Hospice House.
• Work with hospice-eligible patients and their loved ones as they begin admission process in collaboration with the Hospice House staff.
• Develop and implement territory business plans and manage sales time/resources to meet goals.
• Establish and maintain ongoing public relations to increase brand awareness and community understanding of the organization and services.
• Utilize CRM software to effectively manage accounts and performance.
• Performance of other duties as required
• Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies and procedures
