Job summary

AccentCare Inc. is seeking an RN Clinical Team Director - Hospice in Southgate, MI. This full-time role requires 5+ years of RN experience with at least 2 years in a supervisory capacity.

The position oversees direct patient care teams and IDG care planning, with on-site responsibilities and occasional on-call expectations. The salary ranges from $95,000 to $105,000 annually with a Monday–Friday schedule and on-call occasional duties.

Qualifications

• Minimum of 5 years RN experience with at least 2 years in a Nursing Supervisor role.
• CHPN certification is preferred.
• At least one year of supervisory experience.
• CPR certification preferred.
• Licensed RN in state of practice with ability to travel to all locations.

Responsibilities

• Supervise direct patient and family care team members in collaboration with site leadership.
• Manage scheduling, payroll, and other administrative duties.
• Oversee initial assessments and required reassessments within required timeframes and documentation.
• Lead interdisciplinary group (IDG) meetings and support care planning for each patient and family.
• Support staff education and mentor team members in patient care activities and documentation.
