AccentCare, Inc. is seeking a Hospice RN Team Director in Southgate, MI. This full-time role requires an RN with 5+ years of experience and at least 2 years in a supervisory capacity. The position is on-site with coverage in Detroit & DownRiver. Salary ranges from $95,000 to $105,000 per year.

The ideal candidate will hold an Associate degree (CHPN preferred), maintain CPR, and be willing to travel to all service locations. Schedule is Monday to Friday with occasional on-call duties.

Qualifications

• Associate degree preferred.
• Certified Hospice Nurse (CHPN) preferred.
• Three+ years' experience in hospice, home health, general medical surgery, oncology, or geriatrics.
• Minimum of one year of supervisory experience.
• Possess and maintain valid CPR certification (preferred).
• Must be a licensed driver who can travel to all business locations.

Responsabilités

• Supervise direct patient and family care team members, in collaboration with program site leadership.
• Responsible for scheduling, payroll, and other administrative duties.
• Ensure initial assessments and required reassessments are conducted timely and documented.
• Oversee and support the Interdisciplinary Group (IDG) in development and review of care plans.
• Collaborate with site leadership to mentor staff and ensure protocol adherence.
