Job summary

AccentCare Inc in Fall River, MA is seeking a full-time Hospice Registered Nurse Team Director (RN) to lead patient and family care teams. The role includes supervision, scheduling, and ensuring timely assessments within a multi-disciplinary setting.

The position offers a salary range of $95,000–$115,000 per year with Friday on-call administration. A CHPN and supervisory experience are preferred, with travel to various locations as needed.

Qualifications

• Associate degree preferred.
• Certified Hospice Nurse (CHPN) preferred.
• Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (geriatrics, pediatric, etc.).
• Minimum of one year of supervisory experience.
• Possess and maintain CPR certification (preferred).
• Must be a licensed driver who can travel to all business locations.
• Meet regulations and requirements of the state(s) served.

Responsibilities

• Supervise patient and family care team members in collaboration with site leadership.
• Schedule, payroll, and other administrative duties.
• Ensure initial assessments and required reassessments are conducted on time and documented.
• Oversee the IDG plan of care development and review for each patient/family.
• Collaborate with program site leadership to mentor staff in patient care, productivity, and documentation.
• Promote Back to Bedside and reinforce Case Management skills per protocol.
• Support staff education efforts with site leadership.
• Facilitate IDG meetings and provide ongoing IDG support and quality calls.
• Engage in quality improvement activities including data collection.
• Reinforce and support internal and external communications.
