Job summary

AccentCare in Plantation, Florida is seeking a dedicated Registered Nurse Team Director to lead the hospice nursing team. This full-time role requires strong clinical expertise and supervisory experience to coordinate patient care and IDG activities.

The position offers a competitive salary of $90,000–$105,000, a Monday–Friday schedule with occasional on-call, and a comprehensive benefits package. Bilingual candidates are preferred to support diverse patient populations.

Qualifications

• Associate degree preferred.
• Certified Hospice Nurse (CHPN) preferred.
• Three+ years' hospice/home health/medical-surgical/oncology experience.
• Minimum of one year of supervisory experience.
• CPR certification preferred.
• Must have a valid driver's license and be able to travel to all locations.
• Meet state regulations where the program provides services.

Responsibilities

• Supervise direct patient and family care team members with site leadership.
• Handle scheduling, payroll, and other admin duties.
• Ensure initial and follow-up assessments are timely and well-documented.
• Oversee IDG development and care plan implementation for each patient/family.
• Mentor staff in patient care, productivity, and documentation.
• Support Back to Bedside initiatives and case management skills.
• Assist site leadership with staff education and training.
• Facilitate IDG meetings and provide ongoing support and triage.
• Participate in quality improvement activities and data collection.
• Support internal and external communications.
