Job summary

AccentCare is seeking a Hospice Registered Nurse Team Director to lead clinical teams across patient populations with a focus on high-quality, compassionate care. This full-time role emphasizes supervision, care coordination, and policy-compliant administration in a hospice setting.

The ideal candidate will have supervisory experience, CHPN certification preferred, and be willing to travel to multiple locations.

Qualifications

• Associate degree preferred.
• Certified Hospice Nurse (CHPN) preferred.
• Three+ years' experience in hospice, home health, or related nursing.
• Minimum of one year of supervisory experience.
• Possess and maintain CPR certification (preferred).
• Must have a valid driver's license and be able to travel to all locations.

Responsibilities

• Supervise direct patient and family care team members with site leadership.
• Handle scheduling, payroll, and other administrative duties.
• Ensure initial assessments and required reassessments are completed timely by qualified staff.
• Oversee the Interdisciplinary Group (IDG) plan of care development and review.
• Collaborate with program leadership to monitor staff performance and documentation.
• Support Back to Bedside initiatives and reinforce case management skills.
• Assist and lead quality improvement activities and data collection.
