Qualifications

• Associates degree and RN license required in state practicing.
• CPR certification preferred/required in some states.
• Minimum one year of experience in hospice/home health or related nursing preferred.

Responsibilities

• Conducts comprehensive nursing assessments and develops care plans with the IDG.
• Maintains timely documentation of care and patient status.
• Coordinates care activities including visits, meds, and physician collaboration.
• Provides education and support to patients and caregivers; participates in on-call coverage.
