Job summary

Lighthouse Hospice in Downers Grove, IL seeks a compassionate Hospice Registered Nurse - PRN to provide patient-centered nursing care in patients' homes or facilities.

You will collaborate with the interdisciplinary team, assess needs, manage symptoms, educate families, and participate in end-of-life planning while maintaining documentation in the EMR.

Requires RN licensure, CPR certification, and a valid driver's license; hospice experience is preferred.

Qualifications

• Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure: Current state RN license and/or as per Nurse Licensure Compact rules; valid CPR.
• Experience: RN in clinical care; hospice experience preferred.
• Skills: Nursing practice, strong communication, compassionate patient-focused care.
• Transportation: Reliable vehicle and auto liability insurance.

Responsibilities

• Provide skilled nursing care, symptom management, medications, and pain relief.
• Conduct comprehensive assessments of physical, functional, psychosocial, and environmental needs.
• Perform routine and as-needed visits to patients in home or facility settings.
• Educate patients and families on disease processes, self-care, and end-of-life care.
• Collaborate with physicians, social workers, and chaplains as part of the care team.
• Document thoroughly in the EMR and maintain care plan continuity.
