Job summary

Frontpoint Health, LLC is seeking a PRN Hospice Registered Nurse (RN) to conduct initial and ongoing assessments and coordinate end-of-life care for patients in the Dallas area. Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse (RN) and/or in accordance with Nurse Licensure Compact rules.
• Two years’ experience as an RN in a clinical setting; hospice experience preferred.
• Nursing skills aligned with generally accepted standards; good interpersonal skills; palliation/end-of-life experience preferred.
• Must read, write and comprehend English.
• Reliable transportation, valid driver’s license and auto liability insurance.

Responsibilities

• Perform initial and ongoing comprehensive assessments of the impact of terminal disease on patients' physical, functional, psychosocial, and environmental needs, with documentation.
• Apply admission and re-certification criteria to establish appropriate levels of care and eligibility for hospice care.
• Develop and document the plan of care to ensure quality and continuity of care; revise as necessary.
• Educate patients and families about disease process, self-care techniques, end-of-life care, nutrition, and dietary needs; provide training to family and staff as needed.
• Determine scope and frequency of services based on acuity and patient/family needs; assess caregiver capacity.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Provide on-call responsibilities and on-call services as assigned.
