Job summary

Lighthouse Hospice Care LLC seeks a Registered Nurse to administer nursing care to patients in their homes and coordinate with the Interdisciplinary Group. You will assess needs, initiate care plans, and ensure quality care across visits.

Qualified candidates hold an RN license, have reliable transportation, and can read, write, and speak English; Spanish fluency is required. Salary ranges reflect BOE and Illinois market norms.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a registered nurse (RN) and Driver's license.
• Experience as an RN in a clinical care setting; hospice preferred.
• Fluent reading, writing, and comprehension in English; able to communicate with patients and families.

Responsibilities

• Initial and ongoing assessments of patient needs and response to care.
• Develop and implement the plan of care, coordinating with the IDG and patient family.
• Educate patients and families on disease processes and end-of-life care; provide staff training as needed.
• Provide on-call coverage and effective communication with the care team.
