Frontpoint Health is seeking a Hospice - PRN Registered Nurse (RN) to provide skilled nursing care in patients' residences in the Dallas area. The role collaborates with the interdisciplinary team to assess, plan, and implement end-of-life care.

Responsibilities include comprehensive patient assessments, care planning, education for patients and families, and on-call coverage. Requires RN licensure, English proficiency, and reliable transportation; hospice experience preferred.

Formación

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state RN license or in accordance with Nurse Licensure Compact rules.
• Two years RN experience in a clinical care setting; hospice experience preferred.
• Must read, write and comprehend English.
• Reliable transportation and valid driver’s license with auto liability insurance.

Responsabilidades

• Initial and ongoing comprehensive assessments of patients across physical, functional, psychosocial, and environmental needs.
• Coordinate care with the Interdisciplinary Group/Team and families; plan and document the care plan.
• Educate patient and family on disease process, end-of-life care, nutrition and self-care; train staff as needed.
• Determine scope and frequency of services based on acuity and needs; monitor for care continuity.
• Provide on-call services and communicate changes in patient needs to the physician and team.
