Job summary

Apploi is seeking a part-time Registered Nurse for hospice care, delivering patient-centered nursing in home and community settings. The role focuses on assessing patient needs, developing and implementing a care plan, and coordinating with families and physicians to ensure compassionate, high-quality care.

The ideal candidate holds an Illinois RN license, CPR certification, and hospice experience is preferred.

Qualifications

• Graduate of an accredited school of nursing with active Illinois licensure.
• CPR certification required.
• Bachelor’s degree with hospice care experience preferred.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Assesses and initiates a care plan based on nursing diagnoses.
• Regularly re-evaluates patient and family needs and revises the plan of care as necessary.
• Initiates and revises the plan of care as patient status changes.
• Uses health data to guide nursing diagnoses and actions.
• Develops care plans with patient and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing and related needs.
• Provides health education to patient as appropriate.
• Assists with activities of daily living and supports self-sufficiency and comfort care.
• Communicates with physician and documents changes in condition.
