Job summary

Lighthouse Hospice in Downers Grove, IL seeks a compassionate Hospice Registered Nurse for PRN, per hour shifts to deliver patient-centered care in patients' homes. You will coordinate with the interdisciplinary team and support families through the end-of-life journey.

Required are an active RN license and CPR, with hospice or clinical care experience; flexible scheduling and competitive pay apply, with benefits including referral program and 401(k).

Qualifications

• Graduate from an accredited nursing program (Diploma/Associate/Baccalaureate).
• Current state RN license and CPR certification.
• Experience in clinical care; hospice experience preferred.

Responsibilities

• Provide skilled nursing care and symptom management in patients' homes.
• Coordinate care with the interdisciplinary team and family members.
• Educate patients and families on disease processes and care plans.
• Maintain accurate documentation in the EMR and ensure quality of care.
