Alc-Health-Care-Services is seeking a PRN Hospice Registered Nurse to plan, organize and direct home-based hospice care across communities in Downers Grove, IL. The role emphasizes community health education and coordinating patient and family needs during home visits.

Responsibilities include comprehensive assessments, developing care plans, administering prescribed nursing care, and collaborating with families and the hospice interdisciplinary team to ensure comfort and support.

Formación

• Graduate of an accredited school of nursing; 1-2 years acute care; 1-2 years home care.
• Current licensure in State and CPR certification.
• Bachelor's degree; hospice care experience preferred.
• Supervising hospice aides not required.
• Excellent observation and communication skills; basic math.

Responsabilidades

• Completes initial and ongoing patient and family assessments to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Assesses and evaluates patient's status by planning and initiating care.
• Regularly re-evaluates patient and family/caregiver needs and revises the plan.
• Initiates the plan of care and makes revisions as patient status changes.
• Communicates with the physician regarding patient needs and orders.
• Works with family/caregiver and hospice team to meet emotional needs.
• Attends interdisciplinary group meetings.
