Job summary

Hestia Hospice & Family Care, LLC seeks a Registered Nurse to coordinate patient-centered care in the Houston area. The role requires assessment, care planning, and collaboration with an interdisciplinary team to deliver quality home health services under the Nurse Practice Act and physician orders.

Responsibilities include comprehensive assessments, plan development and updates, medication administration, patient and family education, and documentation.

Qualifications

• Graduate of an accredited school of professional nursing.
• Minimum of one year experience as a professional nurse preferred.
• Bachelor’s degree in Nursing preferred.
• Valid RN license within practicing state required.
• Bilingual Spanish reading/writing/speaking preferred.

Responsibilities

• Coordinate patient-centered care with the healthcare team.
• Perform comprehensive initial and ongoing patient assessments.
• Develop and revise plans of care with interdisciplinary input.
• Deliver quality home health nursing per physician orders.
• Administer medications and treatments as prescribed.
• Educate patients and families on care and self-management.
• Maintain accurate clinical notes and report changes.
• Act as case manager for assigned caseload.
• Collaborate with physicians and community resources.
