Job summary

Good Shepherd Hospice is seeking a compassionate Registered Nurse Case Manager to serve patients and families in Houston and surrounding communities. The RN provides nursing assessment, planning, and care to maximize comfort and health, responding to calls, visiting patients, and handling after-hours admissions and deaths as needed.

The role requires licensure as an RN in the state, and hospice/home health/geriatrics experience is preferred.

Qualifications

• Licensed Registered Nurse in the state where care is provided.
• Hospice/home health/geriatrics experience preferred.
• Demonstrated commitment to the hospice philosophy of care.

Responsibilities

• Receives patient assignments and coordinates care with physicians, hospice staff and agencies.
• Maintains regular communication with attending physicians and families.
• Assesses patients and gathers data for the Plan of Care.
• Maintains up-to-date patient records and Medicare documentation.
• Instructs patients and caregivers on medications, personal care and safety.
• Attends care plan and team meetings and provides support at time of death.
• Provides 24/7 coverage to patients and families.
