Job summary

Constellation Health Services is seeking a compassionate Registered Nurse Case Manager to support hospice patients and families in their homes, assisted living facilities, and skilled nursing facilities in the Houston area. You will perform comprehensive assessments, coordinate plans of care, participate in Interdisciplinary Team Meetings, and ensure timely documentation.

Weekend and holiday coverage may be required, with a supportive, family-centered culture and strong emphasis on empathetic

Qualifications

• Graduate of an accredited RN program.
• Licensed Registered Nurse in the state of employment.
• 1–2 years of RN experience required; hospice experience strongly desired.
• Basic knowledge of iOS operating system.

Responsibilities

• Provides skilled nursing care, including administration of medications and treatments.
• Performs comprehensive assessments and coordinates the plan of care with disciplines and caregivers.
• Observes, evaluates and reports patient status to nursing supervisor and physician as needed.
• Participates in weekly Interdisciplinary Team Meetings and ensures timely documentation.
• Weekend & holiday coverage may be required.
